• How do I check the status of my request? To check the status of your request, please sign in to your ChartRequest account or visit app.chartrequest.com/status. Here you can see real-time status updates 24/7.
• Is there any other way to submit records requests besides online? If you cannot go online to create an account, the only other option is to send your authorization and request form directly to support.chartrequest.com. Their team will help you set up an account and submit your request so we can view it and respond in our system.
