Third-party records requests:
Systems Design West fulfills third-party medical, billing, and/or balance verification records requests electronically via our ChartRequest portal. ChartRequest is a HIPAA-compliant record exchange platform that streamlines the records request and retrieval process. To submit a request you must create an account by visiting our ChartRequest portal here and following the signup instructions. This should only take a few minutes.
Once your request is in our system, we will be more than happy to get you the information you need. If you have any questions about the ChartRequest online platform, please visit the ChartRequest website. FAQ’s:
- Who do I address the authorization/release to? All authorizations and releases must be addressed to the transporting provider, not Systems Design West.
- What site location do I use when submitting a request in ChartRequest? Systems Design West.
- How do I check the status of my request? To check the status of your request, please sign in to your ChartRequest account or check your status here for real-time status updates 24/7.
- How do I submit patient records requests? Patient records requests should be sent directly to the provider.
- I don’t see an option to select ‘Balance’ on ChartRequest. How do I submit a balance verification request? Please select “Billing” when submitting your request. The balance verification document you upload in ChartRequest will allow us to determine the specific type of request and process it accordingly.
- Is the price shown when I submit my request the final price? No, the initial price is an estimate and the total will be reflected on the final invoice.
- Is there any other way to submit records requests besides online? If you cannot go online to create an account, the only other option is to send your authorization and request form directly to support@chartrequest.com. Their team will help you set up an account and submit your request so we can view it and respond in our system.
